Berks food safety inspections found old meat and sludge on a shelf and a fly infestation at one store

Berks County is divided into three jurisdictions: Reading, Muhlenberg Township

Berks County is divided into three jurisdictions: Reading, Muhlenberg Township and the rest of the county. Reading and Muhlenberg Township have their own food safety inspectors and the rest of the county is inspected by Pennsylvania Department of Agriculture inspectors.

The results will be posted every other week. All food vendors are inspected at least once a year. Other inspections can be triggered by a change of owner, a follow up if a facility had been out of compliance or by a complaint filed with the PDA.

If a food provider is labeled out of compliance, that means it has one or more violations that require a return visit by an inspector. The establishment is given time to fix the problem(s) and remains open for business.

Below are the results of inspections conducted between Sept. 21 and Oct. 11, 2022, that were filed in the state database as of Oct. 13 at 5 p.m.

Rita's of Hamburg, 400 S. Fourth St., Hamburg, October 02: two violations. The floor / wall juncture in the three compartment sink area is not coved and closed to 1/32 inch. Exit door located in the food prep area of the food facility has a gap and does not protect against the entry of insects, rodents, and other animals.

Arby's #5229, 15506 Kutztown Road, Kutztown, Sept. 29: eight violations. Ladies restroom hot water has very low to no pressure at this time. The Person in Charge did not demonstrate adequate knowledge of the PA Food Code as evidenced by incorrect responses to food safety questions. Floors under all have dirt/grease build up. Also the Water softener is leaking water in corner for an extended time causing a sludge build up and loosened wall tiles in that area. Observed sandwich line equipment, in kitchen area, with an accumulation of dust, dirt, food residue, debris on non-food contact surfaces. Rear door to the outside, located in the kitchen area of the food facility is not self-closing and/or is being propped open allowing for fly/fruit fly infestation. Corrected. Water dripping into overflowing pans under sandwich counter area. Temperature measuring device for ensuring proper temperature of equipment is not available or readily accessible in sandwich drawer equipment or hot hold oven. The floor / wall juncture in walk in cooler and mop sink area is not coved and closed to 1/32 inch. All new coving tiles are gapped and also floor in front of mop sink has a large gap. Drawers in sandwich line with food were being held at 47°F, rather than 41°F or below as required. Items placed on time in lieu of temperature until repaired. Out of compliance.

Autumn Festival At Savage Farms, 1710 State St., Mertztown, Sept. 29: no violations.

Aztlan Mexican Grill, 1665 State Hill Road, Wyomissing, Sept. 29: one violation. New food facility does not yet employ a certified food employee as required.

Aztlan Mexican Ice, 1665 State Hill Road, Wyomissing, Sept. 29: one violation. One set of lights is not shielded or shatter proof in the back area.

Burger King #4750, Rt 222 & Quarry Road, Kutztown, Sept. 29: no violations.

Chipotle Mexican Grill #1546, 2733 Paper Mill Road, Wyomissing, Sept. 29: one violation. There is an accumulation of dirt, old food debris, other items under the equipment in the back area, including under the CO2 tank.

Foodleigh.Com, 342 W. Main St., Kutztown, Sept. 29: two violations. Observed soup food cooling at room temperature on the kitchen which is not a proper cooling method. Corrected with ice bath. Observed clean food equipment and/or utensils in counter area, stored uncovered or not inverted. Corrected.

Mr Food Store #66104, 342 W. Main St., Kutztown, Sept. 29: no violations.

Say Cheese! Restaurant & Cheese, 600 Penn Ave., West Reading, Sept. 29: two violations. Some of the plastic lids for the bulk dry good bins are cracked/damaged. Observed dirt, debris on the floor under equipment at the cook line and in the corners of the dry storage room and chemical room.

Stonersville Hotel, 5701 Boyertown Pike, Birdsboro, Sept. 29: no violations.

Subway #11418, 1193 Berkshire Blvd., Wyomissing, Sept. 29: four violations. Food employee observed in front area wearing multiple bracelets on arms. Mop sink has damaged areas that are no longer smooth, easily cleanable. Some of the caulking is also pulling away from the wall, leaving a gap. A couple soda nozzles and the ice chute at the self-serve soda unit, food contact surfaces, were observed to have residue and were not clean to sight and touch. Corrected. The food facility does not have the original certificate for the certified food employee posted in public view.

501 Grill & Tavern, 7646 Lancaster Ave., Mount Aetna, Sept. 28: four violations. Grease and food debris on floor behind equipment on the cook line. Vacuum-packed salmon was thawed in vacuum packaging which is not an approved thawing method. Discarded. Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was 0 ppm, and not 50-100 ppm as required. Bucket was empty and changed to new bucket. Corrected. Also interior bin of ice machine has slight mold build up along top and sides of interior bin. Shelving racks in walk in cooler have mold along wire shelving areas and wheel areas and must be cleaned. Also beer cooler in basement has mold build up on exterior of beer lines and shelves and flooring area where kegs are stored.

Alebrije Mexican Restaurant, 2224 State Hill Road Wyomissing, Pa 19610, Sept. 28: no violations.

Crumbl Cookies, 2779 Papermill Road, Wyomissing, Sept. 28: five violations. The food facility does not have the original certificate for the certified food employee posted in public view. It is posted in the back room but needs to be in public view. Several of the plastic lids for the bulk storage bins are cracked/damaged. There is an accumulation of dirt, old food debris on the floor under and around the grey storage racks in the back area. The side seal on the back door is damaged to the point that part of it is missing. Food employee personal belongings observed stored on ingredient containers/boxes in the back area, although lockers are provided.

Dee Dee's At The Market, Leesport Farmers Market Po Box 747, Leesport, Sept. 28: four violations. White plastic bins used to store raw onions and potatoes have brown residue. Observed an accumulation of dirt, old food debris on the floor under the mat at the cooking area. An open employee's beverage container was observed in a food preparation area. Corrected. The food facility does not have the original certificate for the certified food employee posted in public view. Food employees observed in food prep/service area not wearing proper hair restraints such as nets or hats.

Perry Restaurant, 11 Bellevue Ave., Shoemakersville, Sept. 28: no violations.

Vida Cafe Brunch & Tacos, 351 N. Centre Ave., Leesport, Sept. 28: two violations. A working container (spray bottle) of cleaner / sanitizer was stored on one of the bain marie units in the kitchen area. Corrected. Working container (spray bottle) in front area, used for storing chemicals, cleaners taken from bulk supplies, was not marked with the common name of the chemical. Corrected.

Wilson School District Senior High School, 2601 Grandview Blvd West Lawn, Pa 19609, Sept. 28: four violations. Faucet head of the Salvajor food disposal is rusted and no longer a smooth easy to clean surface. The quaternary ammonia concentration in the sanitizing solution of the 3-bay warewash sink was 100 ppm, rather than 200 ppm as stated on the manufacturers use directions. Detergent dispenser line on front of mechanical warewashing machine is leaking leaving a detergent residue build up along front of machine and on ends of line. Hot water sanitizing thermometer of the mechanical warewashing machine is cracked and was not functioning.

Wilson School District Sh Lower House, 2601 Grandview Blvd West Lawn, Pa 19609, Sept. 28: one violation. Mechanical warewashing machine has a leak at the pressure valve on top left of machine causing a puddle with lime build up running across top of machine.

Conrad Weiser School District High School, 44 Big Spring Road, Robesonia, Sept. 27: three violations. Temporary wooden threshold between old and new walk in cooler sections to be completed with a smooth easy to clean surface. Warewashing machine has lime build up around drainboard area. Floor tiles are badly cracked at flooring around grease trap at warewashing machine.

Conrad Weiser School District Middle School, 347 E. Penn Ave., Robesonia, Sept. 27: two violations. Warewashing machine has lime build up around the drain board area at rack exit area and also caulking along wall at drainboard has gaps allowing a mold residue build up. Water, as observed at warewashing room hand sink, is not under pressure during inspection. Faucet needs repair so sink is inoperable at this time.

Rise & Grind Nutrition Shillington, 606 Kenhorst Plaza, Reading, September 27: one violation. Plumbing system not maintained in good repair – observed drain line leaking at the warewashing/hand sink area.

Tulpehocken School District Bethel Elementary, 8390 Lancaster Ave., Bethel, Sept. 23: one violation. The hot water used for sanitizing in the mechanical warewashing machine did not reach 180°F (or 165°F for stationary rack). Service call placed and manual warewashing sink to be used for sanitizing.

Tulpehocken School District Jr/Sr High School, 430 New Schaefferstown Road, Bernville, Sept. 23: no violations.

Twin Valley High School, 4851 N Twin Valley Road, Elverson, Sept. 23: three violations. Three-bay sink has a hose attachment that is hanging below the flood rim of the sink and not an appropriate air gap. Corrected. Boyers Food Market #66229, 7166 Bernville Road, Bernville, Sept. 21: 14 violations. The person in charge does not have adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Bulk open bagel bin at front of store is full of flies and fruit flies. Bin removed to discard product. Several raw animal foods were stored above ready-to-eat foods in the aisle display case. Presliced tomatoes, a refrigerated, ready-to-eat time temperature control for safety food in the deli area, was date-marked by the facility, but was beyond the 7-day use or sell-by date and requires discarding. Also found expired milk offered for sale. Corrected. Records at deli for recording slicer cleaning and sanitizing are not filled out completely and have no dates on forms. Bulk bagel stand does not have a placard or signage indicating that allergies and ingredients are available upon request. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Excessive grease and food debris buildup under grill/fryer area. Observed chemical spray bottle at the bar area not labeled. Corrected. Food dispensing utensil in bulk food observed stored in the food and not with handle above the top of the food and the container. Corrected. Observed dust debris build up on walls and ceiling in the food prep area. Observed clean food equipment and/or utensils in the food prep area, stored uncovered or not inverted. Mechanical warewashing equipment had a hard water buildup observed inside. No sign or poster posted at the handwash sink in the food prep area to remind food employees to wash their hands. Zhuang's Garden, 2834 Penn Ave., West Lawn, Sept. 21: four violations. Observed boxes of vegetables stored directly on the floor in walk in cooler area yet, rather than 6 inches off of the floor as required. Also bags of flour/cornstarch, rice etc. stored on floor in dry storage room. Bins used to store flour/corn starch etc. and scoop handles in bins have a heavy old product residue and dirt build up and must be cleaned. Fryer/grill equipment, in the cook line area, is fixed to the floor, but is not spaced to allow access for cleaning on all sides and cannot be moved to properly clean underneath. Observed sides/tops of cooking equipment in the kitchen area, with encrusted grease and soil accumulation. Fixed equipment cannot be moved for adequate cleaning. Sides and top of reachable areas must be cleaned.

Aldi, 3520 N. Fifth Street Highway, Oct. 08: no violations.

Dunkin, 3634 Pottsville Pike, Oct. 08: five violations. Food employees observed not wearing proper hair restraints, such as nets, hats, or beard covers. Kobalt adjustable wrench observed on food prep table, and must be stored to prevent contamination of food. Muffin pans not stored covered or inverted. Ice bin containers not stored covered or inverted. Mops are not being hung to air dry. The front handwashing sink was being used for sanitizing beverage machine components and may only be used for handwashing.

Dutch Maid Bakery, 2934 N. Fifth Street Highway, Oct. 08: one violation. Observed dusty ceiling vents, which may potentially contaminate any exposed baked goods. Personal items, such as phones and purses, observed stored in food preparation areas and should be stored in a designated space that is not a food area.

Salino's Imports Fairgrounds LLC, 2934 N. Fifth Street Highway, Oct. 08: no violations.

Regal Buffet, 3252 N. Fifth Street Highway, Oct. 1: four violations. Observed walk-in refrigerator shelving with plastic epoxy coating cracked and torn, thus it is not a smooth cleanable surface. Shelf liners in cook's walk-in refrigerator are soiled and require cleaning. Raw chicken displayed next to raw shrimp at hibachi station requiring movement over the shrimp with potential cross-contamination. To correct, raw chicken was moved to alternate position immediately during inspection. Dock screen door located in the back area of the food facility has a gap at lower door threshold and does not protect against the entry of insects, rodents, and other animals.

Turkey Hill Minit Market #31, 2240 Hampden Blvd., Sept. 26: one violation. Walk-in refrigerator shelving is soiled in places, and requires cleaning.

V & S Sandwich Shop North Inc., 4447 N. Fifth Street Highway, Sept. 26: two violations. Ice scoop stored in a container that is not able to drain/air dry properly. Mops are not being hung to air dry.

AMC, 3050 N. Fifth Street Highway, Sept. 23: two violations. Ice scoop stored in a container that is not able to drain/air dry properly. Mops are not being hung to air dry.

Grocery Outlet, 3260 N. Fifth Street Highway, Sept. 23: no violations.

J & F Seafood, 2934 N. Fifth Street Highway, Sept. 23: four violations. A Food Employee was observed washing their hands at the 3-compatment sink rather than the designated handwashing sink. Food employees observed in market stand/prep area, not wearing proper hair restraints, such as nets, hats, or beard covers. This is a repeat violation. (2017,2018, 2019, 2020, 2021) Handwashing sink water valves shut off, and not in use due to leak. Water temperature within guidelines, and sink is functional only when turned on. This is a repeat violation. Handwashing should only be occurring in the handwashing sink, and not in other sinks used for other purposes. Floor/wall junction compromised with hole, currently filled with metal scrubbie to prohibit rodent access.
